The game normally prevents you from accessing the building mode in buildings such as hospitals and police stations. I tested the gameplay and there is no problem in career progression. I demolished Willow Creek Hospital and built a modern hospital building. If you are playing doctor career in the sims 4 get a work expansion pack, you will be going to willow creek hospital frequently for work.
